Summary: Kaaren Mathias describes how she started and continued a community mental health project in India where a community team amidst extreme poverty, religious discrimination, a variety of beliefs and treatment approaches to mental health issues, bureaucratic and sometimes corrupt public services, education and discrimination as access barriers, unethical private providers often of low quality, massive inequalities, gender discrimination, stigma and a host of other challenges. In the middle of all this a successful and innovative programme emerged from an OM design that identifies key actors with and the aims of interactions with them.

As well as being deeply embedded at the grass roots Kaaren is a New Zealand/Indian public health specialist, a researcher with many publications, an academic and board member of Health Systems Global.
